Every two years the Utah State Library for the Blind sends out a patron survey. We want to know how you feel about us and your library service. We use the data to make adjustments to our processes and programs to better meet your needs.
You will receive a paper copy of the survey in the SeeNote Newsletter mailing, which you can complete and return in the mail at no cost to you. You can also complete the survey digitally by following the link below. We hope to hear from you!
