Certification is Necessary
To qualify for library service an application should be obtained by either printing the form from the link below, calling or emailing the Library to request a form to be sent through the mail. The application must be signed by a competent authority such as a doctor, ophthalmologist, registered nurse, social worker, case worker, counselor, rehabilitation teacher, or superintendent; professional staff member of a hospital or other institution or agency.
